JOB PURPOSE: As part of the
Trueblood settlement, the state is funding enhanced engagement to connect
individuals detained in city and county jails with services. This position will
provide outreach, engagement, and case management services to individuals who
are involved in the criminal court system that have been ordered to undergo a
competency evaluation, restoration, or participate in diversion services. This
individual will develop a service plan with the client and assist them in
obtaining, maintaining, and/or using appropriate mental health, general health,
and other community resources relevant to their goals. They can expect to
provide peer counseling, outreach, crisis intervention assistance,
stabilization assistance and case management services.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Assist clients to determine which
services are needed, such as behavioral health treatment services,
housing, food, transportation, medical services, and other social services
as identified.
- Support client's staying connected
to services already enrolled in or connect and refer to community
behavioral health and substance use treatment.
- Plans, implements, and coordinates
services required to meet the health and human service needs of identified
priority clients, including housing, food, clothing, access to primary
health care, psychiatric services, and other services as identified.
- Develop and maintain positive
collaborative working relationship with other social services providers,
crisis stabilization facilities, crisis responders, evaluation and
treatment facility staff, DSHS workers, substance use disorder treatment
providers, mental health providers, health care providers, shelter
providers, landlords, harm reduction resources, withdrawal management
program, protective or representative payees, and other community programs
which may support clients.
- Provide wrap around support
to help client secure and maintain appropriate housing.
- Meet
participants where they are physically and mentally at to engage client in
meeting their treatment goals.
- Develop
trusting relationships with participants, promoting a sense of self-direction
and self-advocacy, and helping the participant plan how they will successfully
manage their life.
- Provides ongoing assessment of a client's
cognitive, emotional, physical, social, and vocational strengths and needs.
- Assists in the development of natural support
systems for clients.
- Advocates for
clients with community agencies.
- Teaches skills
such as problem solving, goal setting, money management, symptom
management, communication, social skills, and how to access community
resources.
- Use his or her own lived experience of recovery
from addiction/mental illness plus skills learned in formal training to deliver
services to eligible clients.
- Maintains and submits data and reports as
required.
- Maintains familiarity
with and handles client information in accordance with Federal Regulations (42
CFR, Part 2), the Revised Code of Washington (RCW 71.05.390 and RCW 71.24), and
other applicable laws pertaining to confidentiality of client and staff
information.
- Maintains physical
security of confidential materials and assigned Agency property.
SECONDARY

proficiency in utilization of the Agency's electronic medical record
system.

PERFORMANCE
EXPECTATIONS:
- Demonstrate
ability providing street-based outreach and engagement vulnerable populations.
Street outreach requires the ability to easily navigate city streets on foot
and tolerate a variety of weather conditions. Street outreach experience a plus.
- Demonstrate
experience developing positive, collaborative relationships with criminal
justice system and social services providers to effectively serve individuals.
- Maintain accurate, timely chart information in compliance with funding
source while also meeting agency quality assurance guidelines.
- Meets attendance and punctuality standards necessary for effective
program participant care.
- Meets
all contract obligations.
- Understand and
adhere to state and federal confidentiality regulations.
- Conducts self
ethically and professionally at all times.
- Coordinate
and prioritize work assignments so that tasks are completed on time and in
a quality manner.
REQUIRED EDUCATION,
LICENSE(S), CERTIFICATION, AND EXPERIENCE:
Education: Minimum High School Diploma/GED
Licensure: Agency Affiliated Counselor registration (Peninsula
Behavioral Health will assist in obtaining this credential)
Certified
Peer Support Specialist credential within 6 months of hire (Peninsula Behavioral
Health will assist with obtaining the required training)
Experience: Must
be a person who has their very own personal experience receiving services for a
mental health condition OR is in recovery from a substance use disorder for at
least the last two consecutive years.
Other: Must possess and maintain a valid WA driver's license and a satisfactory and insurable driving record to be
able to drive company vehicles.
Additional
requirements:
- Must
be able to pass a pre-employment drug test and background check
- Must
be able to provide vaccination records for MMR, Hep B, Tdap and recent flu shot
