JOB PURPOSE: Provides leadership and coordination of Outreach
Services inclusive of PATH, Peer Pathfinder and Recovery Navigation programs.
ESSENTIAL
DUTIES AND RESPONSIBILITIES:
- Development and coordination of community partner relationships,
including Law Enforcement, court staff, community referral partners,
community-based organizations, etc.
- Represent the REAL program to community stakeholders using effective communication and strategic partnerships to best leverage the REAL program strengths and contribute to the success of the program.
- Development and coordination of community partner relationships, including Law Enforcement, court staff, community referral partners, community-based organizations, etc.
- Program oversight of all aspects of the Recovery Navigator
Program.
- Advocate for individuals to gain access to a wide variety of
community resources.
- Attend REAL Program Team meetings, training courses, and
regional/state required meetings.
- Tracking and coordinating all purchasing activities with accuracy
and timeliness.
- Facilitate conflict resolution between REAL Program staff.
- Oversight of supervision of REAL Program Project Manager, Outreach
Support/Care Managers and Recovery Coach(es) to ensure that REAL Program staff
are completing job duties in accordance with job descriptions.
- Utilize resiliency practices and trauma informed care practices to
provide clinical support of REAL
program staff with lived experience related to REAL
program participants and/or experiencing secondary trauma.
- As necessary or appropriate, must be able to provide outreach,
referral, and outreach support/care manager to ensure team flexibility during
implementation and sustainment phases of their area's coverage area.
- Ensure outreach, engagement, and coordination supports are
consistent with program policies.
- Facilitate data collection, data reporting and program evaluation efforts.
- Monitor and manage REAL program funds for monthly reporting to
the SBH-ASO.
- Play an active role in required meetings, such as PCG and
Leadership Meetings. Support Project Manager, as appropriate, with additional
required meetings for the REAL program such as, individual county REAL program
meetings, REAL program regional meetings, REAL program Operational Work Group
Meetings (OWP), REAL program Policy Coordinating Group (PCG), and other
required trainings and meetings as requested.
- Support Project Manager in coordination with the REAL program
Administrator regarding the REAL program.
- Maintain familiarity with and handles client
information in accordance with Federal Regulations (42 CFR, Part 2), the
Revised Code of Washington (RCW 71.05.390 and RCW 71.24), and other applicable
laws pertaining to confidentiality of client and staff information.
- Maintain physical security of confidential materials
and assigned Agency property.
- Provide oversight of PATH & Peer Pathfinder services.
- Provide direct leadership to PATH Peer Certified Counselors and Staff within the
Agency.
- Participate in community meetings and providers education regarding PATH services as
assigned.
- Ensure that PATH and Peer Pathfinder services are provided consistent with the PATH
Outreach Contract with Clallam County Department of Health and Human Services,
the State Targeted Response to the Opioid Crisis Grant (SOR), and the Peninsula
Community Health Services Hub and Spoke contract, federal, state, and Agency
standards.
- Provide oversight for department bus passes and naloxone program.
- Ensure all PATH and Peer Pathfinder Program monthly, quarterly, and annual reports are
completed and submitted as required by contract.
- Provide outreach and engagement services as needed to individuals who are homeless and
suspected of Opioid Use Disorders.
- Provide
outreach and establish connections with individuals who are homeless and
suspected of having a SUD (Substance Use Disorder), as needed.
- Establish strong relationships and partnerships with a variety of treatment providers
including emergency room staff, substance use disorder treatment providers,
recovery support providers,
- Provide
direct support services to clients, as appropriate and needed to support the
success of the team.
- Use
his or her own lived experience of recovery from addiction/mental illness plus
skills learned in formal training to deliver services to eligible clients and
support to staff.
- Maintain and submits data and reports as required.
SECONDARY

ability providing street-based outreach and engagement to offenders and
difficult to engage populations. Street outreach requires the ability to easily
navigate city streets on foot and tolerate a variety of weather conditions.
Street outreach experiences a plus.

enforcement and social services providers to effectively serve mutual
individuals.
- Understanding a
recovery orientated perspective which defines recovery from mental health or
substance use disorder as a process of change through which individuals improve

REQUIRED EDUCATION,
LICENSE(S), CERTIFICATION, AND EXPERIENCE:
Education: Bachelor's Degree in
Behavioral Science or related field
Licensure: Agency Affiliated Counselor Registration credential (assistance provided)
Certified Peer Support Specialist credential within 6 months of hire (assistance provided)
Experience: Must
be a person who has direct, personal lived experience receiving services for a
mental health condition or is in recovery from substance use disorder for at
least the last two consecutive years
Other: Valid
Driver's License (must have satisfactory and insurable driving record to be
able to drive company vehicles)
Additional
requirements:
- Must be able to pass a
pre-employment drug test and background check
- Must be able to provide vaccination
records for MMR, Hep B, Tdap and recent flu shot
