JOB PURPOSE:   Provide a range of community based vocational, educational, case management supports and family education services to transition age youth/adults and their families for the New Journeys Program.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Interviews individuals and, if applicable, family members or team members in the client support system to complete the strengths based vocational/educational assessment to identify clients' skills and strengths, interests, goals, dreams, aspirations and resources available to reach their goals
  • Counsels individuals to help them understand and overcome personal, social, or behavioral problems affecting their vocational/educational situations
  • Develops an employment and/or education focused service plans based on clients' interests, abilities, strengths and needs; service plans will be client driven and follow-ups will be completed on time, assessing the quantity and quality of services provided
  • Researches possible job and/or education openings and opportunities for clients by contacting employment/school services, newspaper want ads, previous contacts, cold calling, and any other means of reaching potential employers and/or schools
  • Performs job analysis, matching potential openings with clients' needs, skills, and logistics of getting to the job site
  • Establishes and maintains connections with businesses offering employment opportunities
  • Sets up interviews for clients, assists clients with applications, resume preparation, and interviewing skills
  • Locates barriers to client employment and/or educational goals, such as inaccessible sites, inflexible schedules, case management needs, finances, and transportation problems, and works with clients to develop strategies for overcoming these barriers
  • Maintains close contact with clients during job and educational placements to resolve problems and evaluate placement adequacy
  • Performs interventions as needed with assigned clients previously placed who may be at risk of losing their jobs and/or education
  • Work in collaboration with referral sources, community service providers, and all adjunctive service providers to assess, develop and facilitate individualized treatment services and ensure continuity of care.
  • Provide a broad range of medically necessary case management services within the framework of a community support model; services may be provided in agency sites, schools, homes and other community settings as appropriate
  • Participate in clinical supervision, team consultation, and psychiatric consultations, as scheduled.
  • Participate in monthly consultation calls with the UW New Journeys training team
  • Attend all team meetings including business meetings and case reviews; attend individual service team meetings as negotiated with treatment team
  • Work collaboratively with multi-disciplinary teams; manage individual and shared client caseloads per agency guidelines, develop services appropriate to a community based service model, and participate in the management of program resources within a managed care framework
  • Completes and maintains appropriate records which meet Washington Administrative Code (WAC), Revised Codes of Washington (RCW), Regional contracts, and all other applicable County and Agency requirements.
  • Maintains familiarity with and handles client information in accordance with Federal Regulations (42 CFR, Part 2), the Revised Code of Washington (RCW 71.05.390 and RCW 71.24), and other applicable laws pertaining to confidentiality of client and staff information.
  • Maintains physical security of confidential materials and assigned Agency property.

SECONDARY DUTIES AND RESPONSIBILITIES:

  • Performs relevant clinical and administrative tasks according to agency policies.
  • Demonstrates proficiency in utilization of the Center's electronic medical record system.
  • Participates in team meetings and program development.
  • Attends Performance Improvement Meetings on a regular scheduled basis.
  • Provides information and education to community members.
  • Maintains high standards of professional conduct in interaction with clients, staff and other community members.
  • Maintains professional competency through a program of professional development and in-service training.
  • Demonstrates flexibility in adapting to changing work demands.
  • Performs other duties as assigned or requested.

PERFORMANCE EXPECTATIONS:

  • Meets attendance and punctuality standards necessary for effective client care.
  • Maintains accurate, timely chart information in compliance with State, funding source, and agency quality assurance guidelines.
  • Meets agency productivity expectations of 55% consistently.
  • Coordinates and prioritizes work assignments so that tasks are completed on time.

REQUIRED EDUCATION, LICENSE(S), CERTIFICATION, AND EXPERIENCE:

Education:  Bachelor's Degree Psychology, Social Work or closely related field

Licensure:  Agency Affiliated Counselor Registration (assistance provided)

Experience:  One year experience in providing employment, case management supportive services or clinical treatment services with individuals with serious and persistent mental illness and/or substance use disorders

Other:  Must possess and maintain a valid driver's license and a satisfactory and insurable driving record to be able to drive company vehicles 

Additional requirements:

  • Must be able to pass a pre-employment drug test and background check
  • Must be able to provide vaccination records for MMR, Hep B, Tdap and recent flu shot

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of the classification of mental disorders contained in the current DSM.
  • Understanding of the impact of health related problems on psychiatric illness.
  • Ability to educate clients about signs, symptoms, and recovery from mental illness.
  • Understanding of current knowledge on recovery.
  • Knowledge of social service delivery systems, particularly as related to individuals with serious, persistent mental illness.
  • Ability to coordinate care with multiple service providers. 
  • Ability to multitask and effectively manage a caseload of individuals with diverse needs.
  • Understanding of current knowledge on recovery.
  • Ability to work independently.
  • Ability to communicate effectively both orally and in writing.
  • Capability to use sound judgment in problem solving and clinical intervention.
  • Practice of respectful communication with clients to enhance rapport and positive outcomes.
  • Ability to plan and prioritize work and meet the Center's performance, quality assurance and productivity standards. 

NATURE AND SCOPE:

Physical Demands:

  • Moderate physical effort occasionally lifting up to ten pounds.
  • Must be able to operate a motor vehicle.
  • Must be able to tolerate sitting in a vehicle, office or community setting for the required number of hours in a work day.

Cognitive Skills:

  • Advanced verbal and written communication skills.
  • Must be able to enter data into a computerized electronic medical record.

Working Environment:

  • May be exposed to infections and contagious diseases
  • Occasionally exposed to patients exhibiting assaultive behaviors
  • Able to visit with clients in office and community settings.
