Pay Range: $135,000 to $160,000 per year (DOE/DOQ)

Hours per Week:  40 hours

JOB PURPOSE:  The Primary Care Provider fulfills a key role in our busy Community Mental Health Center, providing comprehensive healthcare services. This full-time opportunity works within a collaborative team of professionals, offering a wide range of services, from health promotion and disease prevention to direct care and counseling. This position works closely with the Medical Director to establish a fully integrated model of care for our patients.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Manages a caseload of patients with acute and chronic disease states with responsibility of overall patient primary care.
  • Conducts physical exams as required.
  • Develops treatment plans for acute and chronic diseases.
  • Educates and guides patients on disease prevention and healthy lifestyle habits.
  • Understands the changes (and challenges) in health promotion throughout the aging process.
  • Performs and interprets diagnostic tests and screening evaluations.
  • Emphasizes preventative care and disease management.
  • Prescribes, administers, and reviews medications of assigned clients. Regularly monitors side effects of prescribed medications. Provides consumers with information necessary to make informed consents.
  • Provide regularly scheduled (usually weekly) caseload consultation to psychiatric peers. Case review consultations focus primarily on those clients seen in both primary care and psychiatry.
  • Develops and delivers in-service training regarding recognition and treatment of physical health conditions in psychiatric patients.
  • Conducts and completes brief health assessments for clients at the Clallam County Respite Center (CCRC).
  • Maintains familiarity with and handles client information in accordance with Federal Regulations (42 CFR, Part 2), the Revised Code of Washington (RCW 71.05.390 and RCW 71.24), and other applicable laws pertaining to confidentiality of client and staff information.
  • Maintains physical security of confidential materials and assigned Agency property.

SECONDARY DUTIES AND RESPONSIBILITIES:

  • Maintains professional licensures and credentials.
  • Participates actively in professional organizations in order to further education, professional growth, and networking.
  • Performs relevant administrative tasks according to Agency needs and expectations.
  • Maintains high standards of professional conduct in relation to clients, staff, and other community professionals.
  • Responds to community needs for regular business hours availability and may provide some evening and weekend consultations as the need arises.
  • Abides by the Policies and Procedures of the Agency.

PERFORMANCE EXPECTATIONS:

  • Monthly supervision following initial training. Higher or closer supervision during training or special projects as determined by Medical Director.
  • Performance is based upon collaboratively determined parameters and improvement in caseload health outcomes.
  • Meeting compliance requirements to ensure ongoing operations is critical. Position is accountable for the compliance and ethical health of the organization.
  • Attends a minimum of 90% of required meetings, staffing, trainings, and educational opportunities as scheduled.

REQUIRED EDUCATION, LICENSE(S), CERTIFICATION, AND EXPERIENCE:

Education:  Degree from an accredited school in one of the following disciplines: Nurse Practitioner (NP), Medical Doctor (MD), Doctor of Osteopathic Medicine (DO), or Naturopathic Doctor (ND).

Continuing Education Credits as required to maintain licensure.

Licensure:  Current, unrestricted licensure in Washington State as a NP, MD, DO, or ND.

DEA Certification for Scheduled I-V Drugs (if applicable).

Eligibility for credentialing with various insurance panels.

Experience:  At least FOUR (4) years of experience in treating acute and chronic disease states is preferred.

Additional requirements:

  • Must be able to pass a pre-employment drug test and background check.
  • Must be able to provide vaccination records for MMR, Hep B, Tdap and recent flu shot.

KNOWLEDGE, SKILLS, AND ABILITIES:

Essential:

  • Knowledge of age-specific care across the lifespan.
  • Ability to assess data reflective of the patient's status and interpret appropriate information to address patient needs.
  • Proficiency in tracking treatment response, monitoring patients for clinical symptoms, and managing side effects or complications.
  • Demonstrate effective leadership and team-building skills.

NATURE AND SCOPE:

Physical Demands:

  • Moderate physical effort (lift / carry up to 25 lbs.)
  • Occasional standing / walking
  • Occasionally lifts supplies and equipment.
  • Required Protective Equipment: maintain Standard Precautions and use barriers (gloves, masks, aprons, goggles) appropriately in all potential exposures to body fluids.

Cognitive Skills:

  • Advanced verbal and written communication skills
  • Effective team building techniques.

Working Environment:

  • May be exposed to infections and contagious diseases.
  • Works in a clean, well-lighted and ventilated office
  • Occasionally exposed to patients exhibiting assaultive behaviors
  • Possible visits to clients' homes.

  • Frequent pressure due to multiple calls and inquiries
  • Contact with patients under a wide variety of circumstances.
  • Subject to varying and unpredictable situations
  • Handles emergency or crisis situations, rarely assist in restraining or de­escalating agitated individuals.
  • Subject to long irregular hours

  • Managing chronic, often complex clinical problems.
  • Maintaining a caseload efficiently.
  • Ensuring compliance with a variety of regulations from multiple funding sources.
